Please help us to ensure that your benefits are assigned by carefully noting the following information! We will provide you with a cost estimate based on your treatment program. Please plan on paying all estimated co-payments and deductibles at your appointment.

Our fees may exceed your insurance company’s allowance (UCR). Since the UCR in many cases is unknown, there may be a balance remaining after treatment. These balances are due within 30 days.

If your insurance company rejects a claim or pays less than expected, the balance is your responsibility and is due within 30 days.

Please provide your benefits information to us before your first visit so that we can verify coverage. If we are unable to verify coverage, please plan to pay in full at your appointment.

Please plan to pay in full at your appointment based on the cost estimate provided.

In addition to cash & personal checks, we also accept Visa, Mastercard, Discover & American Express. We also offer CareCredit®, a healthcare credit card which allows you to begin treatment immediately & pay over time with low monthly payment options (subject to credit approval).
